AI form builder

The AI form builder that leaves you in charge

Say what the form is for, and hootform drafts the questions, the order, the logic and the endings. You get an ordinary draft to change however you like, never a black box.

Live: CSAT survey template — nothing is saved

Starting from a blank page is the slowest part of building a form. hootform can draft the first version from a sentence or two, the way a colleague would, and then help the finished form get better answers than a form usually does.

From a prompt to a form

Choose New form and describe what it's for, who fills it in and anything it must ask. For example:

A feedback form for a cooking class. Ask what they cooked, how they'd rate the class, and what to change next time. If they rate it low, ask what went wrong.

A prompt like that comes back as a draft along these lines: a short-text question for the dish, a five-star rating, a long-text question for changes, a follow-up after a low rating, and an ending that thanks them. Question types are chosen to fit (a rating, not a text box, for "how would you rate"), and choices, logic rules and endings are filled in. Ask for a quiz and the draft is scored, with the result on the ending. See templates and AI.

You stay in control

Nothing is saved until you choose to create the form, so a draft you don't like leaves nothing behind. Once you create it, it's an ordinary form: change any word, question, rule or ending in the builder, add your design and a picture, and publish when you're ready. AI never publishes anything, and never changes a form after you've edited it.

Whatever the model suggests is checked before you see it. Every question, choice, logic rule, score and follow-up is held to the same rules as a form built by hand, and anything that can't be fixed is dropped rather than left broken. So the draft always saves, and anything still missing before it can be published is pointed out.

Follow-ups on vague answers

AI helps after publishing too. When an answer doesn't say enough, like "it was fine", hootform can ask one more question before moving on: "What would have made it better than fine?" You choose which questions may dig deeper, and can write the first follow-up yourself. A follow-up is never required and never changes where the form goes next. The live form beside this page asks one after a low score. See follow-up questions.

Questions people ask

Is the AI form builder free?

Drafting a form with AI and AI follow-ups are part of Premium, at $19 a month for the whole organization. Templates, and editing any form by hand, are free on every plan.

Which AI model does it use?

hootform uses a large language model from Anthropic or OpenAI, depending on how it's configured. Your description is sent to draft the form, and nothing else about your account is.

Can AI add logic to the form?

Yes. Drafts can include logic rules and several endings, which you can review on the Logic tab before publishing.

Can AI make a scored quiz?

Yes. Describe a quiz, a test or a lead score, and the draft gives each right answer its points, shows the score on the ending, and can send a perfect score to an ending of its own. Check the answer marked right on every question before you publish: AI can be wrong about facts. See the quiz maker.

Does the AI see my responses?

Not when drafting a form. It only sees the description you write. AI insights, a separate Premium feature, reads responses but leaves identifying answers out.
