Why these questions
A contact form has one job: get the message to the right person with enough context to reply. This template asks four questions and nothing more: name, email, what it's about and how can we help?
The topic is a dropdown of talking to sales, help with something, billing and partnerships, plus "Other". It costs the visitor one click and saves you from reading every message to route it. The topic is its own column in the responses table and the CSV export, and a webhook can send each message somewhere different depending on it.
It asks one question at a time, which suits a contact form better than you'd expect. Each step is small, so people who start usually finish, and every answer is saved as it's given. Even a message someone abandons halfway isn't lost: on Premium you can read partial responses.
The template also records utm_source and utm_campaign from the page link as
hidden fields. When a sales enquiry arrives, you know which campaign
brought them, with no analytics setup.
Putting it on your site: embed it inline on your contact page, or as a popup or a slider on every page. Getting told: turn on notifications so the team is emailed each message the moment it's sent. Keeping out spam: every form has spam protection built in, with no CAPTCHA for real visitors to solve.

Questions people ask
Can I embed this contact form on my website?
Yes. Open the form, choose Share, then the Embed tab, and paste the snippet into your site. It works inline, as a popup, as a slider or full-window, on any site that accepts HTML.
How do I get an email when someone submits the form?
Turn on email notifications for the form and choose which teammates hear about new responses.
Does it stop spam without a CAPTCHA?
Yes. Every form checks for bots quietly, so real visitors never see a puzzle. Bot submissions are dropped before anything is stored.
Can messages go to different teams by topic?
Send every response to a webhook and route it by the topic's value in your own tools, or sort the CSV export by the topic column.
