Why these questions
A quote request form should ask exactly what you need to name a price, and nothing that doesn't change it. This template is set up for a cleaning company, but it works for any service business that prices by the size and type of the job, from gardening to removals.
It opens with what they'd like a quote for: regular cleaning, a one-off deep clean, a moving-out clean, or office cleaning. That answer decides the rest of the form, through logic:
- Homes are asked how many bedrooms and bathrooms. Only regular cleaning then asks how often, since a one-off job doesn't need it.
- Offices skip the bedroom questions and are asked how big the office is instead, then how often.
Everyone then answers when they'd like to start, their postcode ("so we can check we cover your area"), a name and an email for the quote, and an optional phone number for people who'd rather talk. Contact details come last on purpose. By then they've told you about the job, so asking where to send the price feels natural.
Pricing faster: turn on notifications so a new request reaches you straight away, or send requests to a webhook that works out a price from the answers. The ending promises a quote the same working day. Change it if that's not realistic.
To make it yours: rename the services, change the size questions to what drives your price, such as square metres or number of rooms, and keep the rules pointing each service at its own questions.
How the logic works
Each rule is checked once its question is answered, and the first one that matches decides where the respondent goes next. How logic works
- After “What would you like a quote for?”: if the answer is “Office cleaning”, jump to “How big is the office?”.
- After “And bathrooms?”: if the answer to “What would you like a quote for?” is “Regular home cleaning”, jump to “How often would you like us?”.
- After “And bathrooms?”, always jump to “When would you like us to start?”.

Questions people ask
Can the form calculate a price automatically?
Not inside hootform. Send each request to a webhook, and your own system or an automation tool can work out the price from the answers and email it.
How do I ask different questions for different services?
With logic rules on the service question. Here, office cleaning jumps past the bedroom questions to office size, and only regular cleaning is asked how often.
Can I check whether the customer is in my area?
The template asks for a postcode and explains why. Check it when you price the job, or have your webhook check it against the areas you cover.
Can I put the form on my website?
Yes. Embed it inline on your pricing page, or open it as a popup from a "Get a quote" button.
