Why these questions

A course evaluation is only useful if it tells the instructor what to change before the next run. So this template asks about each part of the session separately, and ends with the question instructors actually act on.

It starts with the pre-work: how useful was the talk people were asked to watch beforehand? It's easy to forget to evaluate, and it's often the weakest part of a course. Swap in your own recording when you reuse the form. Then comes an overall rating for the session, five stars.

Next is a grid: how did each part land? Pace, content, handouts and slides, exercises and the instructor are each rated from poor to excellent. A grid makes five ratings feel like one question, and each row shows the instructor exactly where to focus.

Then comes the question that tells you whether the course worked: how confident do you feel putting this into practice? from 0 to 10. On Premium, anyone who answers below 6 gets one follow-up: "What would have made you feel readier?" You write that question yourself, so it's asked word for word.

Two open questions (the most useful part and what to change next time) and would you recommend it to a colleague? finish it. The welcome screen promises the answers are anonymous. Keep that promise by not adding a name question.

Reading the answers: on Premium, Insights shows the grid row by row and the average confidence, and AI insights pulls the themes out of the open answers.

Questions people ask

When should participants fill in the evaluation?

At the end of the session, before people leave the room or the call. Put the link or a QR code on the last slide. Response rates drop sharply once people have gone.

Can I compare the same course across several runs?

Yes. Use one form per run, or one form with a run hidden field in the link, and compare the exports side by side.

Are the answers anonymous?

The form doesn't ask for a name or an email, and respondents never sign in. Answers are only tied to a person if you add a question that identifies them.

Can I use this for online courses?

Yes. Share the link at the end of the last lesson, or embed it on the course page.
