Why these questions

Paper permission slips come back crumpled, unsigned, or not at all. This form works on any phone, collects a signature on screen, and gives the trip leader one list with every child's medical needs and emergency number on it.

Put the trip's details (the date, destination, times and cost) on the welcome screen. The template leaves a note there for you, so parents read what they're agreeing to before they start.

It asks for the child's name, their class, the parent or guardian's name, and a phone number to reach them during the trip. The phone question has a country picker and stores the number in one consistent format.

Does your child have any medical needs or allergies? is a yes/no. Only a yes is asked for the details and any medication, through logic. That answer is required, because it's the one the trip leader needs most.

May we share photos that include your child? is asked separately from permission for the trip itself. Parents can say yes to one and no to the other, and your records show which.

Then comes a consent checkbox giving permission for the trip, which records the exact wording agreed to, and a signature, drawn with a finger or a mouse and stored with the response.

Keeping it safe: turn on encryption and the phone number and signature are encrypted at rest. The medical answer is free text, which is kept readable, so keep the form in a workspace only staff can open. On the day: export the responses to CSV for a list of every child, their medical notes and the number to call.

How the logic works

Each rule is checked once its question is answered, and the first one that matches decides where the respondent goes next. How logic works

  • After “Does your child have any medical needs or allergies we should know about?”: if the answer is No, jump to “May we share photos from the trip that include your child with the school community?”.

Questions people ask

Is a signature on a form legally valid for trip consent?

A drawn signature records that the parent confirmed, alongside the time they did. Whether that's enough for your school depends on your own policy, so check with your school's office.

Can parents fill it in on a phone?

Yes. Each question fits a phone screen, and the signature is drawn with a finger.

How do I see which children haven't returned the form yet?

Export the responses and compare the list of names with your class list. Send the link again to anyone missing.

Can I reuse the form for every trip?

Yes. Keep one form, update the welcome screen's details for each trip, and add a trip hidden field to each trip's link so responses stay sorted by trip. Or create a new form from this template for each one.
