Why these questions
Send an event survey the day after, while people still remember the details, and make it quick enough to finish on the train home. This one takes about a minute, and it tells you what to keep and what to change for next time.
It opens with an overall rating out of five stars, the number you'll put in the report. Then comes a grid: how did each part go? The talks, the workshops, meeting people, the venue, and food and drink are each rated from poor to great. There's also a "Didn't go" column, because someone who skipped the workshops shouldn't have to invent a rating for them. A grid makes five ratings feel like one question, and each row shows you where to spend next year's budget.
Two optional open questions follow: what was the highlight and what should we do differently. The highlights are your quotes for next year's page. The changes are your to-do list.
How likely are you to recommend it to a colleague?, from 0 to 10, gives you a Net Promoter Score for the event, comparable year on year. Would you come next year? and an optional topics for next time finish it off.
Sending it: email the link to attendees the day after, or put it on the closing slide. If attendees registered through a hootform registration form, send it to the same list.
Reading the answers: on Premium, Insights shows the grid row by row, and the NPS calculator turns the recommend scores into one number.

Questions people ask
When should I send an event feedback survey?
Within a day of the event ending. Response rates fall quickly after that, and so does how much people remember about each session.
Why is there a "Didn't go" column?
So people can rate only the parts they attended. Without it, someone who skipped the workshops has to pick a rating anyway, and that makes your numbers less accurate.
Can I use it for a webinar or an online event?
Yes. Rename the grid's rows to fit, such as the talks, the Q&A, the chat and the sound and video, and drop the venue and food.
Can attendees answer anonymously?
The template doesn't ask for a name or an email, and respondents never sign in. Answers are only tied to a person if you add a question that asks.
