Why these questions

A wedding RSVP has two audiences: the guests, who want it to take a minute, and the kitchen, which needs exact numbers. This template keeps both happy, in a design that looks like a stationery card rather than a form.

It starts with who's replying, "as it appears on the invitation", so you can match replies to your guest list. Then comes the question everything turns on: will you be joining us?

If the answer is no, the form skips everything about dinner and dancing and asks one thing: would they like to leave a note for the two of you? Guests who can't come often want to say something, and nobody should have to answer a meal question for a dinner they'll miss. That's logic: one rule on the yes/no question.

If it's yes, the form asks how many are coming, including themselves (capped at four, so a plus-one can't become a plus-five), what they'd like for dinner from your menu, any allergies to tell the kitchen, and one song that will get them on the dance floor. The song question is optional and it's the one guests remember. Then the form ends, with no note question, since they'll see you on the day.

To make it yours: put your names, the date and the venue on the welcome screen, replace the menu with yours, and set a reply-by date as a close condition so late replies don't arrive after the caterer's deadline. Add a photo of the two of you in design.

Counting it up: the responses table and the CSV export give you a row per reply, with guests and meal choices in their own columns, ready for the caterer's spreadsheet.

How the logic works

Each rule is checked once its question is answered, and the first one that matches decides where the respondent goes next. How logic works

  • After “Will you be joining us?”: if the answer is No, jump to “Leave a note for the two of us?”.
  • After “One song that will get you on the dance floor?”, always finish the form.

Questions people ask

How do I stop guests replying after the RSVP deadline?

Set a close date on the form. After that, the link says the form is closed and takes no more replies. Move or remove the date to reopen it if someone needs an extension.

Can guests choose a different meal for each person in their party?

This template asks for one meal choice per reply, which suits most weddings where couples reply together and a note covers the rest. For a meal per guest, ask each guest to reply separately, or add a meal question for the second guest.

Can I send each guest a personal link?

Yes. Add a guest hidden field and put each guest's name or code in their link. It's stored with their reply, so you know exactly who answered.

Is the RSVP form free?

Yes. Collect as many replies as you need on the free plan. Premium removes the small "Powered by hootform" badge, if you'd rather the form carried only your names.
