Why these questions
A booking form for a restaurant has to be quicker than calling. This one takes about a minute and asks, in order, for everything the front of house needs to seat a table well.
Whose name the table is under, then a number to reach them "only if something changes on the night". Saying why you want a phone number is what gets you a real one. Then when, as a date and time, and how many are dining, from one to twelve. Bigger parties usually need a call anyway.
Then the questions that make a good evening: where they'd like to sit (dining room, terrace, bar or wherever you seat them), whether they're celebrating (a birthday, an anniversary, a business dinner), and anything the kitchen should cook around, pick-any, with "Other" for the allergy that's never on the list. A dessert with a candle on it starts with the occasion question.
A last open question, anything else we should know?, catches the pram, the wheelchair and the proposal.
What this form isn't: a live availability system. It collects requests, and the ending says you'll confirm by text. Turn on notifications so whoever's on shift sees each request as it arrives, or send them to your booking system with a webhook.
Privacy: the phone number and the booking time are encrypted at rest when you turn on encryption.

Questions people ask
Does this form check table availability?
No. It collects booking requests, which you confirm. That suits restaurants that take bookings by phone or email today and want requests in one place. Pair it with a booking system if you need live availability.
Can I stop bookings once a night is full?
A response limit closes the whole form after a number of bookings, which suits a single special evening, like a supper club or a set menu night. For day-to-day bookings, confirm each request yourself.
Can I put the form on my website?
Yes. Embed it on your reservations page, or open it as a popup from a "Book a table" button.
How do I know about new bookings straight away?
Turn on email notifications and each request is emailed to your team as soon as it's sent.
