Question types
Contact info
First name, last name, email, phone number and company, asked together as one question.
Ask for someone's details in one step instead of five: a name, an email address, a phone number and a company, each in its own labelled field. Browsers recognise every field, so most people can fill the lot with autofill.
Settings
| Setting | What it does |
|---|---|
| Fields | Tick each field to ask for it: First name, Last name, Email, Phone number and Company. At least one must be on. |
| Required | Beside each field: whether a complete answer has to include it. New questions require the name and email. |
| Default country | Where the phone number's country selector starts. |
The question's own Required switch says whether it can be skipped. If it's optional, a respondent can leave every field empty and move on; once they fill anything in, the fields marked Required have to be filled too.
What respondents see
A labelled field for each field you ask for, with the first and last name side by side (stacked on a phone). Fields that can be left empty say "Optional". Pressing Enter moves to the next field, and continues from the last one.
- The email is checked like an Email question's.
- The phone number works like a Phone number question: a country selector beside the number, and a
+or00prefix typed in the number wins over the selector. - If a required field is empty, the message names it, such as "Fill in your last name."
The answer
Each field is stored separately, with the phone number in international (E.164) format, such as +447700900123.
- CSV: one column per field, headed "Question – Field", so a CRM or mail merge can take each one as it is.
- Emails and the responses table:
Ada Lovelace, [email protected], +447700900123, Analytical Engines. - Webhooks:
valueis a list offield:valuepairs (first_name,last_name,email,phone,company), andtextthe readable version. - Encryption: always encrypted when the form is, and never sent to an AI model.
In logic
is answered and is not answered only. To branch on someone's email address, ask it as its own Email question.
